Mentor – Mentee Tracking Form

The table below has been developed in order to help us keep track of your meeting times. Please note that subject matter is not documented. However, some of the listings below are labeled for activities that must be completed and documented as part of the three-year provisional requirement and have topics names. Please refer to your Mentoring Guides for suggested activities and requirements. Contract faculty need minimum of 10 hours, then consultation with Committee for assessment. Maximum of 40 hours is allowed.
