PCC > Admissions > Name Change
Name / Address Change

It is the student's responsibility to notify the Admissions & Records Office of any change of name, address, etc., that will affect permanent records. All changes must be done in person at the Registrar’s Office (CC 215) or via fax with signature and documentation. Address changes can be performed by calling the Registrar’s Office at (719) 549-3010 (student ID and PIN required), or in person at the Registrar’s Office.

Changing from Maiden to Married Name
Bring a copy of your new social security card, driver’s license, or marriage license to the Registrar’s Office and complete the Name Change Form.

Changing from Married to Maiden Name
Bring a copy of your divorce decree or other legal documentation verifying the name change to the Registrar’s Office and complete the Name Change Form.

Address Change
Bring photo ID to the Registrar’s Office and complete an Address Change Form, OR call the Registrar’s Office at (719) 549-3010 (student ID and PIN required).
