PCC DENTAL HYGIENE PROGRAM APPLICATION CHECK OFF LIST
______ The Dental Hygiene Program uses the National ACT exam as
part of our point system and is required to be taken by all
applicants. There are absolutely no exceptions to this requirement.
**The writing/essay portion of the exam is not required.**
A minimum score of 20 on the reading component of the ACT test
is required for consideration for eligibility for admission to the
program. The reading, science, and composite scores are also utilized in admissions scoring. Test scores must be taken within the time frame of February 1, 2011 and December 31, 2011. You may take the ACT test
more than one time to see if you improve your score although this is up
to the individual applicant. Your highest set of ACT scores will be utilized
for the application process. Scores need to be reported directly from ACT to the Dental Hygiene Department. Student copies will not be
considered as official documentation.
THE SCHOOL CODE FOR PUEBLO COMMUNITY COLLEGE DENTAL HYGIENE DEPARTMENT THAT SHOULD BE PLACED ON YOUR
ACT APPLICATION IS 4741.
The ACT examination is not offered at Pueblo Community College. Arrangements for taking the ACT examination may be made through your high school, college, the American College Testing Program, Post Office Box 414, Iowa City, Iowa 52243, or by registering online at www.act.org

______ One official copy of high school transcript or official GED scores with official seals. These transcripts MAY NOT be hand-carried or mailed in by applicant, but must be sent from the institution to Pueblo Community College Dental Hygiene Program.
______ One official copy of current transcripts (showing grades for courses completed Fall 2011 and courses enrolled in for Spring 2012) from all college/universities attended. These transcripts MAY NOT be hand-carried or mailed in by applicant, but must be sent from the institution to Pueblo Community College Dental Hygiene Program. To be considered for admissions, a minimum cumulative GPA of 2.5 for all combined general education and prerequisite transcripts is required. PCC transcripts do not need to be requested, they will be prepared by the Dental Hygiene Administrative Assistant.
Please Note: College transcripts will be evaluated for transfer credit during program admissions beginning February 1, 2012 and will only be officially evaluated after the Dental Hygiene Application is complete. Evaluation of course transfer can be reviewed unofficially during advising with program faculty.

After students have been accepted to the Dental Hygiene Program, an application for Admission to Pueblo Community College will need to be submitted (this can be done online on the PCC home page). You do not need to fill out the application if you are already attending Pueblo Community College or any other community college in Colorado.
It is the applicant’s responsibility to see that all the application materials have been received by the program no later than February 1, 2012. Incomplete applications will not be considered or reviewed by the Dental Hygiene Admissions Committee.
