EVALUATION PROCEDURES: |

The supervising instructor will assign a grade based on evaluations completed by the student, employment supervisor, and the instructor. These evaluations will be based on the performance objectives agreed upon in the training plan. The instructor will visit the job site and conduct interviews with the student and the supervisor at least twice during the term of placement--more frequently if deemed necessary. The student will schedule a weekly appointment with the instructor to discuss the student's progress. |

Written evaluation sheets will be provided by the instructor and developed from the objectives agreed upon in the training plan. The first type of evaluation will be a weekly report of activities and levels of performance. It will be prepared by the student and the supervisor and mailed or delivered to the instructor weekly. The second type of evaluation will be an overall rating of the student's performance. The student and the employment supervisor will each submit these evaluations midway through the placement and at the end of the placement. The final evaluation sheet will be utilized in assigning the grade. The student and employer will indicate the final grade on their evaluation forms. |

In addition to the work performed as on-the-job training, the student will submit one report to the instructor. This report will be at least three and no more than five double-spaced pages in length. The topic should relate to the work being done at the placement site and must be approved in advance by the instructor. For dates of submission of the report, please see the instructor. |
